Two part time Social Workers are based at St John’s Hospice to offer invaluable support to patients who have long-term or life-limiting illnesses.
Practical Support
We can offer practical advice, support and information on a wide range of resources, which include:
Benefit checks and filling in benefit forms, housing issues, legal matters, funeral directors, applying for a blue badge, signposting to local authority and other relevant agencies, advocacy and letter writing on the patients behalf.
Supporting Patients prior to their discharge from Hospice
Prior to discharge the social work role is to work alongside the patient, their family and carers and other hospice professionals to facilitate a safe discharge, should it be in your home or a residential or nursing establishment.
Psychological and Emotional Support
When someone close to you has a serious illness, for some individuals the diagnosis can be painful and difficult to deal with. We can offer the patient and their family emotional and psycholigical support, by providing an opportunity to talk about your concerns and ensuring you are provided with support in a confidential setting.
